1) Vendor needs to provide influenza vaccination clinics service to the government authority located in Williamsburg, VA.
– Clinics were conducted on a voluntary, walk-in basis, and employees could come during
any time during the established time block.
– Registration forms were provided by the previous vaccination clinic Provider, and each employee would complete the form and provide insurance information on-site prior to receiving vaccinations.
– The Clinic Provider would bill insurance providers directly for the service. If an employee did not have insurance coverage, the Provider would charge a nominal fee to be paid by the employee.
– The Service Provider is responsible for disposing of all medical equipment and waste per federal and state codes, procedures and regulations. Service Provider shall ensure
that all medical equipment and waste is removed from the site prior to leaving a clinic and is notdisposed of on the premises.
– Service Provider shall provide a refrigerated storage place for vaccines if needed.
– Service Provider will offer and maintain an online pre-registration and sign-up for employees to
receive their flu vaccine and will also accommodate walk-in appointments at all flu clinic locations during the established timeframe.
– The Service Provider may provide communications for clinics including toolkits, flyers, signage, patient education materials, etc. at no additional charge . All communication materials
would be subject to prior approval and will be disseminated to sites through Health Services department.
